PAW Fingerprint Services is an authorized Live Scan Service Provider to fingerprint Level 2 Background Checks for FDLE, FBI and all State and Federal Agency's in Florida. Clients must present a valid ORI#, VECHS#, or OCA#. We are AHCA, DCF and DOH photo compliant. The following charges include state and federal taxes as well as PAW processing fees:

• DCF: Background Screening (Department of Childrens and Families) for personnel in Childcare, Mental Health, Disabled Care, and Summer Camp programs $87.00/person. Client must provide a valid ORI number issued by the regional DCF office.
• VECHS Program: Background Screening (Volunteer & Employee Criminal History System) for qualified personnel providing some type of care placement services for children, the elderly or disabled $57.00/person for volunteers and $67.00/person for employees. The fingerprints are retained by FDLE and they are a part of the Clearinghouse database.
• DOEA: Background Screening (Department of Elderly Affairs) for Aging Network Providers & Volunteers, and Long Term Care Volunteers $87.00/person. Client must provide a valid ORI number issued by the regional DCF office.
• AHCA: Backgroud Screening (Agency for Healthcare Administration) for all Healthcare providers $95.00/person photo included. Client must provide a valid OR number issued by the regional DCF office. The fingerprints are retained by FDLE and they are a part of the Clearinghouse database.
• DOACS: Background Screening (Florida Department of Agriculture and Consumer Services) for Division of Licensing, Security Gaurd (D&G) License, Private Investigator (C or CC License), Pawn Broker and all other F.S. 493 applicants  $67.00/person.
• DBPR: Background Screening (Florida Department of Business and Professional Regulations) for Real Estates Sales and Brokers, Real Estates Appraisers, Mortgage Loan Originator, General Contractor, Community Association Managers, Alcoholic, Beverages and Tobacco, Home Inspectors, Mold Remediation or Assessment, Talent Agents, Althletic Agents, Employee Leasing, Slot Machine and Ship Brokers License $67.00/person.
• DOH: Background Screening (Department of Health) applicants for MD, RN, LPN, ARNP, CNA, Massage Therapist, Pharmacist, Psychologist and all other medical licenses $93.00/person photo included. The fingerprints are retained by FDLE and they are a part of the Clearinghouse database.
• DHSMV: Background Screening (Florida Department of Highway Safety and Motor Vehicles) for motor vehicle, mobile home and recreational vehicle dealer licenses, Commercial driving school instructors, agents and employees  $67.00/person.
• Clerk of Circuit Court: Legal Name Changes for petitioners in Miami-Dade and Broward County Courts $67.00/person. Expungement or sealing of record on fingerprint card or form $25.00.
• Guardianship Programs: In Dade and Broward county (Professional and Non-Professional) $67.00/person.
• Federal and State Banking/Investment: For permits and licenses $25.00 for hard card fingerprinting. (Standard FBI Applicant fingerprint card (FD-258)
• Medicaid: Background Screening $95.00/person. The fingerprints are retained by FDLE and they are a part of the Clearinghouse database.
• Florida Office of Financial Regulation: Background Screening for Consumer Collections Agency or Persons $67.00/person.
• Standard Ink or Non FDLE submission Fingerprinting: FBI Applicant card (FD-258) $25.00 for first card, $20.00 for second and $15.00 for each additionl card.
• APD: Background Screening (Agency for Persons with Disabilities) for CDC, Medicaid Waiver Support Coordinators and Group Homes. Client must provide a valid ORI number issued by the regional office $95.00/person.
• Medical Marijuana: $67.00
• Florida Department of Education: $69:00
• Internal Revenue Services: $67:00
• Security Exchange: $67.00
• Florida Notary Services: We notarize Clerk of the Court documents, Real Estate documents, Wills, Deeds, Power of Attorney documents, Affidavits for Moral Character, Medical forms, Travel Consent forms and Acknowledgements. Fees are $10.00 for a first signature and stamp and $5.00 for each additionl signature and stamp.
